Those providers will bill separately for their services rendered and may or may not participate with the same health insurers or health maintenance organizations as Snead Eye Group.

Patients and prospective patients should contact our insurance department for a more personalized estimate of charges and other information. The price may differ due to complications or treatment due a patient’s personal health condition. To help expedite your request to obtain an estimate, when calling, please provide us with the following: Name, Address, Date of Birth, Insurance Company Name and ID number.
Patients and prospective patients should contact each health care practitioner who will provide services to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider.
